1. Re-order Repeat Medication
Please note that only current repeat prescriptions are issuable.
This system still requires 48 hours to process: the online system is simply to make putting in your request more convenient.
If you need to order extra medication or in larger than usual quantities, use the paper system.
It is vital when ordering medication to ensure that you fill in the “medication request notes” with the details of where the prescription needs to go.
To pick up medication from a Pharmacy or our Chilcompton Dispensary please put one of the following into the medication request field:
Dispense, Lloyds opp Martins, Lloyds at St Chad's, Lloyds Paulton, Elm Tree, Clements, Midsomer Pharmacy or Bath Pharmacy (ticking this box means that you have arranged for your medication to be delivered to your home).
Please note Dispensing is only available to patients living more than a mile from a Pharmacy
To pick up a signed script from a branch please specify
St Chads, or Chilcompton
Please put these details as the first word and don’t use any variations of your own, they may cause confusion! If you have any other notes for us about the prescription, they can go after the Chemist/Dispensary or Branch name.
2. View your medical record
You are now able to view certain parts of your medical record, including coded information such as diagnosis codes from 1st April 2016. To request this please call into the surgery and complete a request form - you will need to bring with you some form of identification such as a passport or photo driving licence together with a recent utility bill. For those who already have online access, you can request access via SystmOnline. Alternatively, you can download a form and information using the links below.
Please note that you will only be able to view those items recorded since 1st April 2015 when this new service became available. Should you wish to view anything before this date then please write to the surgery to request this in the normal way.
Test results are not available on-line as these require interpretation by a GP and until such time as there is suitable support information for patients, these will not be included in the service.
